NOMINATION PROCESS
- The nomination shall be done by any of the USM faculty, staff, fellow alumni, batch mates, or alumni chapters.
- The nomination shall be done in writing using the prescribed form. (here’s the link of the forms: https://tinyurl.com/yadthtrx ) This must be accomplished and duly signed by the nominator/s with the following attachments:
- Nominee’s most recent Curriculum Vitae
- Diploma or Transcript of Records
- Certificate of Appointment for the positions held in the different organizations.
- Evidence of the Continuing, Graduate, or Post-Graduate Education
- Evidence of the community involvement (e.g. written documentation, pictures, video, etc.)
- Other relevant supporting pieces of evidence (e.g. research, paper presentations, publications, newspaper articles about the nominee, awards, recognitions, citations, patents, etc.)
- Any additional information or supporting documents.
NOTE:
Duly accomplished nomination forms, with the accompanying required documents, shall be submitted through email or mail on or before September 15, 2025 to:
